Association Between Absolute Neutrophil Count And Mortality In Acute Ischemic Stroke With Pneumonia Complication

Abstract


Acute ischemic stroke with pneumonia complication would be occur inflammatory reaction and lead to neutrophils invasion in lung tissue as the body's defense mechanism against infection process. Absolute neutrophil count (ANC) is related to mortality of acute ischemic stroke patients with complications of pneumonia.

This was a prospective observational cohort study in November 2013 - April 2014 to 34 acute ischemic stroke patients with complication of pneumonia in Dr. Kariadi Hospital Semarang. Correlation test was conducted to determine if the absolute neutrophil count (ANC) is associated with mortality. Correlation test was also performed to determine if the absolute neutrophil count (ANC) associated with NIHSS score at 7th day diagnosis of pneumonia on surviving patients. The cut-off point 7.600/μL from previous studies was used to determine the magnitude of the relative risk of ANC in association with mortality.

Absolute neutrophil count (ANC) was positively correlated with mortality and showed the likelihood of mortality in acute ischemic stroke patients with pneumonia. Absolute neutrophil count (ANC) was also positively correlated with NIHSS scores among surviving patients. Used the cut off point 7.600/μL from previous studies, 85.7% sensitivity, 40.7% specificity, with a non-significant p value 0.203 and rho worth 0.224 was obtained.

Absolute neutrophil count (ANC) was positively correlated with the mortality of acute ischemic stroke patients with pneumonia complication, but it was not statistically significant. It was related to the role of neutrophils in the inflammatory response, in which excessive response can adversely affect the output of mortality and neurological deficits status of patients as measured by the NIHSS score.
